Insurance Option
Preferred Provider/In Network with most insurance plans (most popular listed): Aetna, Cigna, First Choice Health, Health Care Management, Kaiser PPO, Lifewise, Optum Health/United Healthcare, Premera (Blue Cross), Regence (Blue Shield), and Uniform Medical.
As each insurance carrier can vary, it is strongly encouraged that you contact them directly and ask if I am a "Preferred Provider/In Network" with your plan. Also, ask them about your copays, coinsurance, and deductibles to avoid any financial hardships. Contact me if this seems overwhelming I'm happy to assist you.
Not accepting: Medicare, Medicaid/Apple Health, or L&I Insurances. ​

Private Pay/Sliding Scale Option

Standard same-day payments are $90 per hour.

Sliding scale option is available upon request, based on income. Reviewable every three months.

Cash, Credit and Check Option

Cash: please bring exact change as only limited cash is kept in office.

Credit: All major credit, debit, and health saving cards are accepted through Square credit processing service.

Check: please make check payable to "Fore Counseling Services." There is a $35 fee for returned checks.
